Understanding the Cause of Substance Use Disorder

Substance use disorder has been debated for decades by professionals, families, and individuals seeking answers to one of society’s most devastating problems. Two broad viewpoints dominate the discussion: the biological or disease model, which argues addiction is primarily a chronic brain disease, and the moral or personal responsibility model, which emphasizes human choice, accountability, environmental influence, and the ability of a person to change their life through education and personal rehabilitation.

A person is ultimately responsible for the choices they make, even when those choices are influenced by difficult circumstances, trauma, stress, emotional pain, or social pressure. Many people do not begin using drugs because they want to destroy their lives. They often start because they are overwhelmed by problems they do not know how to handle effectively. Emotional pain, anxiety, grief, depression, physical pain, or unresolved personal struggles can drive someone to seek temporary relief. Drugs and alcohol may appear to provide an escape or release from those pressures. The larger the problem the substance seems to solve, the more value the individual may place on that substance. Over time, the person can become dependent on the temporary relief the drug appears to provide.

Social and environmental influences also play a major role. Young people are especially vulnerable to peer pressure and cultural normalization of drug use. A teenager surrounded by friends who use substances, or raised in an environment where drug use is common or socially accepted, is more likely to experiment. What may begin as curiosity or social acceptance can quickly become habitual behavior.

As people grow older, life presents additional challenges. The loss of a loved one, divorce, heartbreak, financial hardship, career disappointments, trauma, isolation, or countless other struggles can leave individuals searching for relief. Millions of variables can influence someone’s decision to use drugs or alcohol, but the reality remains the same: addiction eventually leads to one of three outcomes—sobriety, jail, or death.

Today, much of the public conversation focuses almost entirely on how substances chemically affect the brain. While drugs certainly alter brain function, reducing the problem solely to brain chemistry can remove accountability and hope from the individual. Treating addiction exclusively with medication — often using synthetic substances similar to the drugs the person was already dependent upon — may only manage symptoms without addressing the root causes that led the individual to use drugs in the first place.
